The video describes the surgical strategy to approach a craniopharyngioma with endoscopic technique in a 5-year-old girl. The patient suffered of a severe progressive headache without any neurological deficits. After a detailed description of the preoperative assessment and imaging results, a step-by-step video of the operative technique is offered. A post-operative MRI showed total removal of the tumor, and the histology confirmed as craniopharyngioma adamantinoatous. The patient presented post-operative panhypopituitarism and developed obesity in the long term. No evidence of recurrence at 5 years follow-up
