As part of a significant health service reform agenda, four Aboriginal and Torres Strait Islander Coordinated Care Trials were conducted across five sites in three States and Territories of Australia. The aim was to achieve a more coordinated approach to the delivery of health care services to people with a diverse range of complex and chronic health care needs. The trials were located in Katherine (NT), the Tiwi Islands (NT), Wilcannia (NSW) and Perth/Bunbury (WA). Evaluation, at two levels, was an integral component of the trials. Local level evaluation was designed to determine the effectiveness of each trial within its specific context, and a national evaluation was designed to pool data and review collective progress against national objective |
