Summary A traumatic hyphema is the accumulation of blood in the anterior chamber of the human eye following ocular injury. Management continues to evoke much controversy among practicing ophthalmologists, and for good reason. Anecdotal experience has done more to formulate therapeutic opinions than any one study. This is not to suggest a lack of adequate research, but rather a reflection on the natural history of generally favorable outcomes regardless of management. This module limits the discussion of ocular injury to blunt nonpenetrating trauma. Topics include evaluation and management of a traumatic hyphema, the current controversies surrounding use of antifibrinolytic therapy, and surgical treatment options for medically unresponsive traumatic hyphemas
