Summary "Improving Adherence in Cardiovascular Care is a toolkit designed to raise awareness among health professionals (GPs, practice nurses and pharmacists) of the issues affecting patient adherence to cardiovascular medicines. The Heart Foundation's Quality Use of Medicines Adherence toolkit is an initiative of the Heart Foundation Pharmaceutical Roundtable. The development of this resource was supported by funding from the Australian Better Health Initiative: a joint Australian, State and Territory government initiative. The content was developed by a team at the University of Sydney, led by Associate Professor Parisa Aslani and Professor Ines Krass. The project has been guided by the governance of a steering committee and by advice from an expert advisory panel. Improving Adherence in Cardiovascular Care comprises six modules with accompanying case studies and tables. The resource features tips on how to establish practice systems, modes of communication, and practical evidence-based interventions. This toolkit is designed to help health professionals better communicate with patients, improve their recognition of non-adherence, and increase awareness of the factors that contribute to it. The importance of collaboration between health professionals is also emphasised, to maximise the likelihood of improving adherence to medicines.
