Summary This book describes geriatrics practice models that are used to guide the care of older adults, allowing seniors to remain at home, prevent functional disability, and preserve their quality of life. The models include specific interventions which are performed by health care teams to address the needs of older persons and their caregivers. These models respect patient values, consider patient safety, and appreciate psychosocial needs as well. The book is presented to readers based on the setting of the care of the older individual: the hospital, the outpatient setting, the transition from one site of care to the next, and the emergency department. The editors further present additional models which address the needs of vulnerable populations, and community- based models of care. Finally, this book describes several promising and innovative programs which represent best practice strategies for older persons.
