In 2008, The Centers for Medicare and Medicaid Services (CMS) announced that it would no longer pay for additional healthcare-related costs associated with many preventable errors, which the agency believed could be prevented by following evidence-based guidelines. It further stated that the institution where the event occurred was not allowed to bill the patient for the cost of care related to that error. This program will describe these events, which CMS has defined as ' ... those that should never happen ... '
