Mango Medical is a small business that has experienced rapid growth in rural areas of Hawaii by offering an innovative primary care practice model. This model was built and launched in the turbulent wake of the passage of the Patient Protection and Affordable Care Act (aka "Obamacare") in 2010, the greatest expansion of U.S. government involvement in healthcare since the enactment of Medicare and Medicaid in 1965. This case invites students to analyze potential growth strategies that capitalize on the early financial success of Mango Medical in the current system that pays physicians for each service provided (i.e., volume-based payment) in the unique and challenging market of rural Hawaii while positioning the company to thrive in a future system that pays physicians for value of service, rather than just volume
