Assessment. Sam, (adolescent) bipolar I disorder, current or most recent episode manic, severe. Medication compliance. V3, With good insight and poor judgment / Symptom Media
Sam is feeling good since she stopped taking her medications. Her mood is elated and she has difficulty sitting still for the interview. Sam understands that not taking her medications is the wrong thing to do and has caused her mother to worry but instead raises the possibility that the medication is not working. When asked when she stopped, Sam is unable to answer. Taking the medications makes Sam feel hollow, which led her to stop in the past. In the last instance when Sam went off her medication, she acknowledges she made mistakes that were embarrassing to her and led to consequences at school. Recalling this event makes Sam emotional and irritable. Though Sam worries what she might do when in another manic state, to her it is preferable to the numbness she feels when complying with the medication
